**Videos must include the performance you wish to present in the CAC Variety Show in its entirety. Excerpts, clips or compilation musical/acting/film etc. reels will not be accepted. Videos should not exceed 6 minutes in length**

-I am of age, I have my guardian's permission to participate, or that I have been given permission to sign on behalf of the individual(s) I am submitting for. 

-I give permission for CAC to use this submission to promote the CAC 2026 Variety Show in print, online and social media (CAC will always give credit to artist).

-This is a community event for folks of all ages, and I understand that CAC will prioritize featuring family-friendly acts & content.

-This is the repertoire I wish to share, and I will rehearse & be prepared to present if accepted to participate.

-I understand that unapproved changes to my repertoire offerings (ie. a last minute, on stage song choice swap) will result in my participation being canceled. 

-I understand that my participation is on a volunteer basis and there will be no compensation for my participation; and CAC will not require payment from me or my group to participate.
