In the QPR, we will ask you to self-report your completion of Program Requirements. You will also have a chance to report any additional information that you believe the program should know when considering any incomplete program requirements.

Examples: medical appointments, death in the family, personal issues, medical issues (including COVID-19), etc. 

Please provide as much information are you are comfortable with and please feel free to list "medical issue" instead of any diagnoses. If your circumstances affected multiple program requirements you can write "see above" in the second or third program requirements.

Note:  Information provided may factor into any decisions made by the program including changes to standing or status
