Animal Assisted Therapy for Schizophrenic Patients
Dear Participant,

This letter is a request for your participation in a research project to assess the treatment of anhedonia within schizophrenia using animal assisted therapy. This project is being conducted by Jordan Shomo, a junior enrolled in the College Board’s AP Capstone program (specifically the AP Research Course) under supervision of Phil Caskey (contact at pcaskey@k12.wv.us). Your participation in this project is greatly appreciated and will take approximately 15 minutes to complete.

Your personal information will be kept strictly confidential under legal penalty. Your responses will be processed as part of the aggregate. You must be a minimum of 15 years of age to participate. You will not be asked any information that will implicate your identity in your responses, nor will any data be traceable to you as a participant. Your participation is completely voluntary. You may skip any question and you may discontinue at any time. You will not be affected in any way regardless of your choice to participate or not. University High School’s Institutional Review Board (IRB) acknowledgement and approval of this project is on file, and for any further questions or concerns, please contact Phil Caskey.

I hope that you choose to participate in this project, as it may prove beneficial to understanding the effects of animal assisted therapy on schizophrenic patients. Thank you very much for your time and consideration. If you have any questions about this letter or the research project, please feel free to contact Jordan Shomo at (304) 906-5575 or by e-mail at 560031304@boe.mono.k12.wv.us.

Sincerely,
Jordan Shomo
