I agree that by participating in physical exercise, running or training activities with CT Sculpt Inc. I do so entirely at my own risk. Because physical exercise can be strenuous and subject to risk of serious injury, I understand that a physical examination from a doctor before participating in any exercise activity or using any exercise equipment is recommended. I understand that coaching services or training plans may involve strenuous physical activity accompanied by risk, including but not limited to competitive running training, muscle strength, endurance training and potentially other forms of physical activity. I agree that I am voluntarily participating in these activities and assume all risks of injury, illness, or death.
CT Sculpt, or any persons affiliated there within, are not responsible for any loss of my personal property. This includes any claim for or arising out of any instruction or lesson given which was broadcast, rebroadcast, disseminated, or made generally available to the public, without a fee and which audience had not entered into an agreement with the insured for such instruction or lesson prior to or at the time of the instruction or lesson.
Any recommendation for changes in diet including the use of food supplements, weight reduction and/or body building enhancement products are entirely my responsibility and I will consult a qualified professional prior to undergoing any dietary or food supplement changes.
I acknowledge that by signing this form, my email address will be added to the CT Sculpt email list to receive monthly updates and information, and I understand that I can unsubscribe at any time. Furthermore, by signing this waiver, I consent to CT Sculpt's use of my image in marketing materials, on the website, or in social media content, unless I explicitly indicate my desire to decline.
I acknowledge that I have carefully read this waiver and release and fully understand that it is a release of liability. I agree to release and discharge CT Sculpt, the trainer or instructor from any and all claims or causes of action and I agree to voluntarily waive any right that I may otherwise have to bring a legal action against CT Sculpt, the trainer or instructor for personal injury or property damage. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence. By checking "I agree to the above," I acknowledge that I understand the content and that this release cannot be modified orally.
