This form is for college visits (Juniors and Seniors only), medical needs and funerals for family members. This form is not needed for school-related absence. In order to be EXCUSED, this form and a parent note (email is fine) must be completed at least 3 academic days prior to the absence. Missed work will be given at the teacher's convenience. Failure to follow these procedures may result in an unexcused absence.
