Middle School Registration
This form is to be completed by the Team Coach ONLY.  All teams are identified by their team name.  All teams must be accompanied by at least one coach.  The coach will be assigned to one of two support roles (Question Reader or Scorekeeper) for the Competition.  Please fill out a separate form for EACH team.  

By submitting this form, I verify that I am registering as a Coach of a Middle School Level Team and the students on my team are in Grades 6-8, or as approved by myself.   I also understand that I am responsible for reviewing the Competition Guidelines and understand that it is my responsibility to respond to any inquiries from Competition Coordinators in a timely manner, as well as to advise Competition Coordinators immediately if my teams to withdraw from the competition for any reason.
