Instructions: This application is for the Canadian Badlands Passion Play and Christ the Redeemer Catholic Division which is the educational provider. Please fill in each section of the application and submit it (your typed name on the signature lines will constitute your legal signature).

    III Use of Personal Information (FOIP)

    We are asking for your authorization for the following: Permission to use pictures taken on school site or at the Passion Play as part of student/ program pictorial recording, advertising and media promotions. Permission for media to publish interviews relating to the Summer School program. Release of photograph, student name, address, telephone, email information for Student Directory/Year Book. Permission to list student name in performance programs. I have read and understand the use of the personal information as listed above.

    IV Passion Play Volunteer Waiver

    I recognize that my role as a Passion Play volunteer is important and vital. Because of this, I agree to the following: (I) I recognize any decisions made about my volunteer involvement is done in concert between the individual and the leadership team according to the needs of each year. All casting decisions are made by the director according to availability, suitability, required positions, previous experiences, and flexible and positive attitudes. I agree to give both the Play and my role the quality effort that they, and I, deserve. I agree to give the role my best and at the same time not to partake in any disruptive or unsafe behaviors. I also agree to keep a positive and cooperative attitude toward my fellow actors/ actresses, as well as toward the Production team. (II) I grant permission to the Passion Play Society and any other authorized media group to record, publish, broadcast, and/or distribute any pictures or motion video that I may be in during my involvement in the Passion Play. I understand the Passion Play may use the footage and my image at their sole discretion and without further permission. I do not expect to receive any remuneration or credit. I understand that the Passion Play or other authorized parties are not liable for any damage, loss, injury, or defamation of character resulting from my participation in the Passion Play. I agree to allow my name to be printed in the Passion Play program and/or in other print or electronic media (i.e. the internet). (III) The Passion Play Production Team is deeply concerned with the health and well-being of our volunteers. As such we have certified first-aiders on staff, as well as have first-aid kits. Although the Passion Play team is there to help in emergency situations, I agree to take care of my personal health, including my acknowledgement of the following: I accept that I need to bring all of my prescription medications, should I have any. I accept that I need to bring protection from the weather, including (but not restricted to) appropriate clothing, sunblock, rain gear, safe foot-wear, water, etc. I recognize and accept that no representative of the Passion Play society is authorized or certified to give me any medications, including for (but not restricted to) asthma, diabetes, pain killers, etc. I recognize that the Passion Play site does contain areas that can be dangerous, resulting in a risk of injury or even death. I commit to accepting these risks, and will not hold the Society responsible in the event of my injury and/or death. I recognize that this document is the policy of the Passion Play Society and will be kept confidential. (IV) During my involvement as a cast member of the Passion Play, I may be asked to partake in organized stage combat or other activities on or off stage that could lead to injury. These activities will be lead by trained individuals who may or may not have the proper certification required. Although the Passion Play follows precaution when it comes to their volunteer actors, I recognized the fact that I may be injured during my involvement with the society. In signing this, I release the Canadian Badlands Passion Play Society of any liability if such a case does arrive. I also understand that if I feel unsafe doing any activity, I do have the option of refusing to take part in it at my discretion. I am pleased to volunteer my services to the Canadian Badlands Passion Play Society and will abide by its policies and procedures. I shall hold the Society safe and harmless in case of personal injury arising out of my volunteer duties, if such should arise. I also waive my legal recourse against The Canadian Badlands Passion Play Society in case of injury or death.

    V Tourism Excursion Trips

    Students are invited to take part in day trips to tourist attractions in the Canadian Badlands on July 11 and Monday, July 18, 2016. Bus service will be provided by Christ the Redeemer Catholic School Division. Students will be involved in hiking in the Badlands as well as visiting various indoor and outdoor exhibits, interpretive programs and recreation areas. Participants will be responsible for wearing appropriate clothing, sunscreen and insect protection.

    NOTE: A $75 non-refundable application fee is due with registration. Payment information will be sent by email once your registration has been processed. All remaining fees must be received no later than June 1, 2016. Online payment information (School cash online) will be sent to you once registration is confirmed. If sending a cheque, please make it payable to "St. Anthony's School." Fees may increase each year.
