AP Request Form 2021-22

All 9-11th grade students are required to take a minimum of 6 classes (SMUHSD Board Policy).

In consideration of the health and well-being of our students, any student interested in taking 4 or more AP classes will need to submit a request, with parent approval, to do so. This form will be used to indicate your top three preferred courses should we not be able to fulfill your request.

The deadline for turning in this form is Friday, March 26, 2021.
