Health and Safety Representations: I certify that my child is physically fit to participate in jiu-jitsu activities and does not have any medical condition that would increase their risk of injury. I agree to inform the instructors of any pre-existing medical conditions, allergies, or limitations that my child may have.

Rules and Instructions: I agree that my child will comply with all rules and instructions provided by the instructors for the safety of themselves and others.

Waiver of Liability: I, on behalf of myself, my child, our heirs, executors, administrators, and assigns, hereby waive, release, and discharge Scaredy Cat Jiu Jitsu, its owners, instructors, employees, and agents from any and all claims, demands, actions, or causes of action for injuries sustained to my child or myself while participating in jiu-jitsu classes or related activities.

Emergency Medical Treatment: In the event of an emergency, I authorize Scaredy Cat Jiu Jitsu and its staff to obtain medical treatment for my child, including transportation to a medical facility, if deemed necessary.

Parent/Guardian Agreement: I agree to supervise my child at all times outside of scheduled jiu-jitsu classes and understand that Scaredy Cat Jiu Jitsu is not responsible for providing childcare services.

Photography and Likeness: I grant permission for Scaredy Cat Jiu Jitsu to use photographs or videos taken of my child for promotional purposes without compensation.

Hygiene

In consideration of being allowed to participate in jiu-jitsu classes and related activities at Scaredy Cat Jiu Jitsu, I, the undersigned, agree to the following:

Personal Hygiene: I understand the importance of personal hygiene in jiu-jitsu and agree to ensure that my child arrives for classes clean and wearing appropriate attire.

Gi Maintenance: I agree to regularly clean and maintain my child's jiu-jitsu gi (uniform) to prevent the spread of bacteria and ensure a safe training environment.

Infectious Diseases: I understand that jiu-jitsu is a physical contact sport and involves close proximity to other participants. I agree to notify the academy immediately if my child develops any contagious skin condition, such as ringworm or impetigo, and will keep my child away from classes until the condition is fully treated and no longer contagious.

No Makeup: I agree that my child will not wear makeup during jiu-jitsu classes. 

Removal of Piercings: I agree that all piercings must be removed before participating in jiu-jitsu classes. This is to prevent injuries to both the participant and others during contact activities.

Miscellaneous: This agreement shall be governed by the laws of Canada. If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue to be binding.

I have read this waiver and fully understand its terms. I understand that I am waiving substantial legal rights by signing and that I sign freely and voluntarily.
