RHMS GT Referral Form 2020-2021
Gifted and talented students are those students whose abilities, talents and potential for accomplishment are so exceptional, or developmentally advanced, that they require special provisions to meet their educational needs. In accordance with state and national guidelines, the DCSDf Gifted Education Team has developed a process to gather data in order to match appropriate programming to identified student need.

Components of identification for gifted programming:

Student Aptitude
Student Achievement
Student Performance (or conversely, high ability/underachieving students)
Student Interest
Observable Behaviors (classroom)
Observable Behaviors (home and community)

If you would like to initiate the Gifted Identification Process for your child, please complete this form. Be advised, you will need approximately 30-45 minutes to complete the entire document. There is not a way to save your progress in this form so please be ready, with your child, to fill it out. At the end of this form, there is a section for the student to complete. Please encourage your student to take his or her time as he or she answers these questions.

Parents/Guardians: by completing this form, you are giving permission for the gifted education team to gather all necessary data in this process, including but not limited to, additional assessments such as the Cognitive Abilities Test (CogAT), student interview, portfolio of work, and tests of achievement.

**If you are a student nominating yourself please fill out the General Information portion of the form and the student survey. Please ask your parents/guardians to fill out only the Parent/Guardian survey.

Once this form is complete and submitted, your school's Gifted Education Coordinator, Emily Manuel, will be notified. Mrs. Manuel will contact you within 10 school days of receipt of this referral to update you on the process.
