For the request to be considered complete, you must
1. Fill out the following form completely.
2. Email a copy of any receipts to the HaSTA treasurer (Michele_Kuwaye@notes.k12.hi.us). The treasurer may ask you to also mail original hard copy receipts via USPS if needed.
The HaSTA president will review complete requests. If your request is approved, the president will authorize the treasurer to move forward with payment/reimbursement.
The president will notify you of the status of your request. If you have not received a notification within 7 business days, please follow up with the president (firstname.lastname@example.org) or president elect (email@example.com).
For multiple payments/reimbursements, please fill out an request for each.
