This is a survey to find out what students know or would like to know about the School Based Health Center. It is totally anonymous! The info you provide here will help the SHAC (Student Health Advisory Council) get information out to students about how to use Health Center and what the Health Center offers.
Do you need to tell your parents or get parent permission to come to the health center?

Will any information I share at the health center be kept private from the school?
Yes! The information you share about yourself and the services you receive at the health center is totally private

I'm not sure, but I'm worried about going to the health center at school due to privacy reasons
