Were you evacuated for one night during the explosion on Woodman Ave?

The OEVCA and community partners who are working to collect, organize and distribute the cash, goods and services that have been so generously donated are creating a contact list of everyone who was evacuated for one night. Although we have a good sense of which properties were evacuated, we do not have contact information for everyone.

We are hoping that one person from each household will fill out this form.
