School of Dentistry Event Kit Request Form
Please fill out this form to request items from the event kit available to faculty, academic appointees, and staff for use at conferences, community clinics, and other outreach initiatives. Some notes:
Please first consult this spreadsheet to determine availability. Items are allocated on a first come, first serve basis.
– Please request items at least one week in advance. Ben Alkaly will then outreach you to coordinate pickup from his office (available on Mondays, Wednesdays and Fridays).
– Pickup, return and any shipping logistics are the sole responsibility of the "loanee," not the SOD Communications team.
– Items are expected to be returned in the condition in which they were loaned.
* If items get dirty, consult Ben before attempting any cleaning.
* If an item breaks while on loan, the renting unit is expected to purchase a replacement. Your unit's business manager will be provided an LVO with the cost, and an FAU number will be requested.
