Mac OPT/STEM OPT Supplement
You must complete this form BEFORE your appointment with an ISP staff member to review your OPT or STEM OPT (after you complete 12-month OPT) application and to issue a new I-20 to include with your application.

NOTE: Your OPT or STEM OPT application must be received by USCIS within 30 days of I-20 issuance; the consequence for not doing so is rejection of your OPT or STEM OPT application.

This email address will be reported to your SEVIS record and you will get an email after OPT or STEM OPT approval to activate your SEVP Portal.
Program End Date *
Program End Date may be different then Macalester's Graduation Date. Please answer with month and year, e.g., December 2050, January 2050, May 2050, or August 2050. Find Program End Date on your I-20, page 1, Program of Study section.
Post-Mac U.S. Address
If your U.S. mailing address used on your OPT or STEM OPT application changes while waiting for your application to be processed, you must update your USCIS account as soon as possible. Failure to do so means you will not receive any correspondence from USCIS, and your approved EAD will be "undeliverable."
Also, as a separate step, please update ISP with any address changes while your application is being processed via ISP's SEVIS Update Form. Keep in mind that address changes made to your SEVIS record will not transfer to your USCIS application, so you must update USCIS directly. 
Include U.S. street address (with apartment number), city, state and zip code.
Consent to Open Mail *
If you do not have a valid post-Mac mailing address, we recommend you use ISP's mailing address (c/o ISP, 1600 Grand Ave, St. Paul, MN 55105) to ensure proper delivery of your Employment Authorization Document (EAD) or any Request for Evidence (RFE) from USCIS. ISP will notify you when we receive any mail. Selecting Yes below authorizes ISP to open your mail.
Do you have another SEVIS ID#, or did you take a leave of absence during your time at Macalester? *
SEVIS ID# is on the top page of 1 and 2 of your I-20.
Mailing OPT/STEM OPT Application within 30 Days *
I understand that I must submit my new I-20 recommending OPT or STEM OPT with my application AND within 30 days of the issuance date of new I-20. Failure to submit the required I-20 means that USCIS will deny my OPT/STEM OPT application.
