This is a
one-time form that grants general consent for your young person to take part in Youth activities and tells the Youth team the important information we need to know for your young person.
When you have signed this, you do not need to sign another.The permission you give is a general consent for your young people to take part in regular Youth activities (e.g., Sunday groups and clubs). If you complete a booking form for an event or trip or another one-off event, there will be some additional questions on the form, but this will shorten the booking process.
Please note: whilst this will shorten event/trip booking processes, it is YOUR responsibility to keep your young person's details up to date in detail on MyChurchSuite or by informing Adrian or Abby of any changes (e.g., medication, dietary requirements or allergies, addresses, etc.)
Please fill a separate form out for each of your young people.
