Notice of Collection: Personal information on this form is collected under the authority of sections 58.5(1) and 169.1-173 of the Education Act, R.S.O. 1990, c.E.2 (the “Education Act”) and will be used by Toronto District School Board (“TDSB”) for the purposes of sharing information and resources for Parent/Caregiver engagement. This information is retained in accordance with the Municipal Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. M.56 will be disclosed to authorized TDSB staff to administer the above purposes.
