Field Trip Request
Instructions
Field trip request for should be submitted as soon as possible.

Timeline:
> At least 10 business days prior to trips with no funding requirements.
> At least 30 business days prior to trips funding requirements.

Process:

Step 1: Fill out the following form.

Step 2: Support staff will get all necessary approvals and signatures.

Step 3: When field trip (and funding) has been approved, support staff will email instructor with confirmation including pre-filled permission slips for students. (DO NOT USE YOUR OWN)

Step 4: Instructor prints and signs the permission slip, then copies and distributes to participating students.

Step 5: Instructor scans and submits copies of permission slips to cteworks@placercoe.k12.ca.us prior to departure.

Step 6: Instructor emails (cteworks@placercoe.k12.ca.us) the day of with number of students attending.
