PIRATE BIRTHDAY TREATS
The Department of School Nutrition Services can provide birthday treats to your child's class in the elementary and intermediate schools.  The birthday student will be presented with a birthday card, special treat and a bookmark from us.
This service is NOT available for students attending Pierce Middle School, Merrillville High School or in our Head Start/Preschool program.

Please complete the form below. The order must be placed at least two weeks in advance.  All treats must be paid in advance prior to classroom delivery.

Please have the funds available on your child's Skyward account two days before order is to be delivered.  
All treats are peanut free; however, they are manufactured on shared equipment that processes peanuts, coconut and tree nuts.

You will receive an email from the School Nutrition confirming your request within 48 hours. If you do not, call 219-650-5300 ext. 6089
