Rise Together: Mentor Sign Up Form
As a Rise Together Pathways Mentor, I would be expected to:

1. Attend and participate in one of the program orientations.
2. Commit to a weekly day and 1.5 hour time slot for the 8-10 week program that consistently fits in your schedule. 
3. Participate in 30 minute training sessions/ workshops per week so I am prepared to provide individual guidance to a RT high school mentee. For convenience, training sessions are conducted asynchronously or before/after mentoring sessions. (The 30 mins is included in the 1.5 hour time slot)
4. Guide 8-12 one hour weekly mentoring sessions for your mentee (during the fall semester). The mentor would provide 24 hours notice if they are unable to attend a mentoring session.

4. Maintain confidentiality of all mentee information, including but not limited to their transcript (if applicable), essays (if applicable), letters of recommendation (if available), email contact information, and financial aid information (if disclosed to the mentor) so Rise Together can help work on College Applications, Financial Aid Scholarships, and other post secondary attainment applications. 
5. Fill out the Mentor- Debrief form every week so Program Directors know how to best support you and your student. 
7. Maintain confidentiality of all Rise Together curriculum and materials.

 Your participation and leadership in this program is greatly appreciated. Please feel free to share the application link with other college level students who would also like to volunteer as a mentor.

Commitment Statement: I commit to serve as Mentor for the full 8-10 weeks of programming. My total commitment would be approximately 18 hours for the 8-10 weeks. I understand that I will dress and act professionally at all times, when communicating with Rise Together staff and other mentors/ students, and during sessions. If I display behavior or dress that is unprofessional or unbecoming of a mentor, I understand that I may be asked to leave the program.
