Office of Student Success Summer 2024 Requests for Tutoring
By completing this request, you agree that you are committed to receiving tutoring for the course listed below for the semester. Please fill out one request for EACH course for which you would like to receive a tutor. For any questions about this form, please email tutoring@mail.naz.edu.

PLEASE NOTE: We do not offer tutoring for ENGW courses, MTH courses, or English Language Proficiency.  We offer only limited tutoring for PHL 101, and this is only if students have tried the Logic Lab but the hours don't fit with their schedule. The same is true for foreign language tutoring as we require students to first try the Emerson Language Lab.
 
All requests for ENGW (Writing) tutoring should be directed to the Writing Center.
All requests for MTH tutoring should be directed to the Math Center.
All requests for PHL 101 tutoring should be directed to the Logic Lab.
All requests for foreign language tutoring should be directed to the Emerson Language Lab.

All requests for assistance in study skills, time management, and other academic skills not related to specific course content should be directed to the Office of Student Success.

We are aiming to provide in-person tutoring as much as possible this year, so the expectation is that all tutors will provide tutoring in person. However, we recognize that there may still be students who would prefer to receive tutoring virtually in some circumstances. If we were able to provide a choice, which tutoring format would be your preference? *

Also note that labs are considered separate courses (i.e. BIO 150 and BIO 150L need separate requests). Please submit a separate request for each course for which you would like a tutor.

Statement of Understanding 1 *
Being paired with a tutor is a commitment. Once paired, you agree to meet with your tutor for a minimum of 1 hour each week. Additional details are found in the Tutoring Guidelines Document.
Statement of Understanding 2 *
Tutors are not responsible for grades or for completion of homework. Once paired you agree to come to each tutoring session prepared with questions to ask. Additional details are found in the Tutoring Guidelines Document.
Statement of Understanding 3 *
Communication with your tutor is a requirement. Once paired you agree to communicate regularly and give at least 24 hours notice if you need to cancel a tutoring session. If you no show a meeting for 3 consecutive weeks, or 5 times total, you will be required to meet with the Assistant Director of Tutoring and Student Success before returning to tutoring.
