* You may request reimbursement for fall 2019 by submitting this form by November 29, 2019 if you are on an installment plan. The reimbursement will be applied to your spring installment on 12/1/19. Otherwise you may submit form for both seasons by June 15 and reimbursement will applied to your fall 2020 season if you are returning. A refund will be processed if you are not returning. Please note that you will need to submit one form per player.
