Whitehills Student Directory 2019-20 Permission Form
The Whitehills School Association compiles a student directory for parents, guardians, and students to use to set up playdates, discuss classroom or school news, and generally stay in contact. In order to distribute the directory the WSA needs permission to include your information.

By completing this form, you are giving your permission, as parent or guardian for the child(ren) listed below, for the information you provide to be published in this internal directory. The directory will be distributed to school families in a one-time e-blast and will not be published or available online.

Please complete this form with as much information as you would like published no later than Friday, September 20, 2019 to ensure inclusion in the directory. The form is also available in PDF on the WSA website (http://www.whitehillsparents.org) or as a hard copy in the welcome packet that will be sent home with all students the first week of school.
