PN Level 1 Certification - Grad Rebate Request Form

Congratulations on completing the PN Level 1 Nutrition Certification! If you’re eligible for the rebate, please fill out this form to request your $200 USD rebate.

The graduate rebate will be issued only in the period between program completion and ONE MONTH FOLLOWING the specified program completion deadline, and only after all program payments have been completed. If you are on a payment plan, all outstanding payments must be made before the rebate is issued.

The deadlines are as follows:

  • For students who registered during the March 2025 promotion, the program completion deadline is Sept 30, 2025. The deadline to submit this form is Oct 31, 2025.
  • For students who registered during the May 2025 promotion, the program completion deadline is Nov 30, 2025. The deadline to submit this form is Dec 31, 2025.

Please ensure that all information provided is accurate to avoid delays in processing your request.
