STUDENT ABSENCE, LATE, AND EARLY DISMISSAL FORM
In compliance with government regulations, and to satisfy the school regarding the safety of your child/children, it is imperative that we have a note of explanation regarding the absence.
All absences and lates must be reported to the school office via online form no later than 8:15 AM the day of and early dismissals must be reported as soon as possible, the day of. This form can also be filled out for past absences in the school year that were forgotten or not reported.

To facilitate this process, please complete this online form if any of the following apply:
  • Late arriving (after 8:15 am)
  • Full day or more than one day absences
  • Early dismissal (state time your child needs to be dismissed)
  • If more than 1 child is away, please complete a separate form for each child.
