Oregon Long Term Care Sector Diversity, Equity, and Inclusion (DEI) Survey
The Oregon Health Care Association (OHCA) is your trade association, and the largest long term care trade association in Oregon. You are a member of OHCA through your employer. You can read more about OHCA at
OHCA has hired Engage to Change LLC, an anti-racism/anti-oppression consulting firm, to do an assessment of the diversity, equity, and inclusion (DEI) needs of the long term care sector. This survey is the first piece of that work. Engage to Change (ETC) is conducting this survey and is looking for information from long term care staff about the ways that racism, transphobia, ableism, sexism, homophobia, classism, and xenophobia may show up in your work and workplaces and how OHCA can help.
Your answers to this survey will be kept anonymous and confidential. Only ETC will have access to the individual response data.
If you have any questions, need this survey in another language, and/or have accessibility needs, please reach out to Rakeem and Kasia with ETC at
Unlike some of the engagement surveys you may have taken in the past, the Diversity and Inclusion Survey is unique. It is centered around researched constructs of Inclusion, such as fairness, belonging, and voice. This survey also asks important, though sensitive, demographics like gender identity, race/ethnicity, and sexual orientation. It should take less than 10 minutes to complete.
REMINDER: Please be sure to click "submit" on the final page of this survey or your responses will not be recorded.
