This application is required to volunteer with the San Gabriel Valley Conservation Corps (SGVCC). By completing and submitting this application, you acknowledge and agree to the following:
Liability and Health Disclaimer - As a volunteer of the organization, you agree to abide by the organization's policies and procedures. You understand that you are volunteering at your own risk and that the organization, its employees, and affiliates cannot assume any responsibility or liability for any accident, injury, or health problem that may arise from any volunteer work you perform for the organization
- You represent that, to your knowledge, you are in good health and suffer no physical impairment that would or should prevent your participation in volunteer activities
Permission to Use Photograph/Video- You grant SGVCC, its representatives, and employees the right to take photographs or video of you and your property in connection with the above-identified subject. You authorize San Gabriel Valley Conservation Corps, its assigns, and transferees to copyright, use, and publish the same in print and/or electronically.
- You agree that SGVCC may use such photographs/videos of you with or without your name and for any lawful purpose, including, for example, publicity, illustration, advertising, and web content.
