Dear Applicant,
Please use this Google Forms to notify your recommenders that you have selected them to submit a recommendation form for your application. Please ensure that your recommenders have agreed to provide a recommendation for your DLSU application.
Please make sure to input their details correctly and accurately (Name, Official Designation, and Email Address).
After filling out this Google Forms, they will automatically receive a notification with the instructions on how to fill out the recommendation form and how to submit it.
NOTE: It is the applicant's responsibility to coordinate the recommendation forms to his/her recommenders. The recommendation forms should be submitted by the following:
- Guidance counselor from Grade 11 or 12
- High school principal or subject teacher from Grade 11 or 12
