I,

desire to provide volunteer services at Susannah Wesley Community Center
(SWCC). By signing below, I acknowledge and agree to release and hold harmless
SWCC and its directors, officers, employees, and agents from any and all
liability, claims, or demands that may arise in connection with my volunteer
activities. This agreement applies to all terms, conditions, and
acknowledgments outlined in this document, including but not limited to those
related to participation, liability, safety, and conduct. This release of
liability shall take effect on (Today's Date) and shall remain in effect for the duration of my involvement with SWCC.
1.
Voluntary Participation: I
understand that I am participating in volunteer activities at SWCC of my own
free will and without compensation or benefits. I acknowledge that I may
decline to participate in any activity that I feel is unsafe or that I am not
comfortable with.
2.
Waiver and Release of Liability: I,
the Volunteer, release and forever discharge SWCC, including its successors and
assigns, from any and all liability, claims, or demands of any kind, whether in
law or equity, arising from the services I provide to SWCC. I understand that
this release includes, without limitation, any claims for bodily injury,
personal injury, illness, death, or property damage that may occur during or as
a result of my volunteer work.
3.
Insurance: I
understand that SWCC does not provide health, accident, or worker’s
compensation insurance for volunteers. I acknowledge that I am responsible for
my own insurance coverage, if any.
4.
Medical Treatment & Emergency: I
hereby Release and forever discharge SWCC from any claim whatsoever which
arises or may hereafter arise on account of any first-aid treatment or other
medical services rendered in connection with an emergency during my tenure as a
volunteer with SWCC.
In
the event of an injury or medical emergency, I authorize SWCC to seek emergency
medical care on my behalf. I understand that I am responsible for any resulting
medical costs.
5.
Assumption of Risk: I
acknowledge that volunteering may involve certain risks, including but not
limited to physical activity, lifting or moving materials, interacting with the
public, and potential exposure to illness or injury. I voluntarily accept and
assume these risks as part of my participation. I understand that it is my
responsibility to communicate any physical limitations or concerns before
engaging in any activity. Additionally, I am aware that I should use utility
carts when transporting items weighing more than 10 pounds to help ensure
safety and I understand back belts came be provided at my request.
6.
Confidentiality Agreement: As
a volunteer with our community center, you may become aware of individuals or
families who receive services through our program. To
protect the privacy and dignity of those we serve, volunteers must maintain
strict confidentiality. Sharing or discussing any personally identifiable
information — including, but not limited to, names, addresses, phone numbers,
or services received — with anyone outside of Susannah Wesley Community Center
staff is strictly prohibited.
7.
Code of Conduct: I
agree to conduct myself respectfully, responsibly, and in alignment with SWCC’s
mission. I will follow all safety rules, instructions, and policies provided by
SWCC staff. I agree to support and maintain a respectful, safe, and inclusive
environment for all individuals at Susannah Wesley Community Center. The
following conduct is not tolerated, including but not limited to:

Violating agency
policies, procedures, or worksite rules.

Engaging in
dishonesty, theft, falsification of records, or misuse of agency property.
·
Using inappropriate
language or gestures; engaging in harassment, discrimination, or displaying
offensive logos, graphics, or materials.

Wearing inappropriate
attire that does not align with the organization’s standards of
professionalism, safety, or respect.
·
Physically harming or
attempting to harm others.
·
Possessing or using
alcohol, illegal drugs, firearms, or other weapons without authorization.
·
Intimidation,
coercion, or interfering with the performance or well-being of others.
