This form must be completed no later than midnight on Friday, January 5, 2018.
Cost is $10 per team.
Submission of this form will generate an email invoice that you can submit to your school treasurer.
Invoices and payments should be postmarked by Saturday, January 13, 2018.
On Sunday, January 14, 2018, participating sponsors will receive an email with the link for the team roster form.
Team rosters must be submitted via that form no later than Wednesday, January 17.
Cancellations must be made by Wednesday, January 17 in order to receive a refund.
