Please fill this form out with as much information possible about the event you are planning. Please remember this is a request, not a confirmation and we will do our best to accommodate all your needs for the event.

Event Information: Please give the event name, a detailed description of the event, and an estimate of how many people you think will attend. *
Event Date and Times: The date(s) of the event, please also include dates before or after the event for setup, clean-up, or preparation. Please also list times for all dates. *
Event Location: If the event is not being held on the Cornerstone campus, please list the location the event will be held. *

Advertising: Will you need this event advertised in the Worship Guide, Website, or Facebook? If so, please write a brief blurb for the event. This can be done at a later time, but please remember to get it to us no later than 2 weeks before the event and no later than the Wednesday before for the worship guide.
Audio/Visual Request: Please check all that apply for your event.
Childcare Request: Do you need childcare for your event? If so, what dates do you need childcare, what time should it begin and end, how many preschool, how many elementary? Will you be providing food or snacks for the kids in childcare? Please be as specific as possible. Our Childcare Coordinator, Sharon Schulte, will use this information to provide childcare for the event. If requesting childcare please give notice at least a month before the event, or we may not be able to secure childcare.
Kitchen Use: Will you need the kitchen for your event? Please remember if you use the kitchen you are responsible for all clean-up. Please also list what you need to use in the kitchen (refrigerator, coffee maker, pitchers, etc..)
