Once you have completed this form, you will be sent an email providing you with the information about your appointment for a SOZO session. If you are not able to have your appointment in Pietermaritzburg, then you can email Wendy at wendy@therock.org.za to refer you to a Sozo team closer to you.

I acknowledge that team members from His People Church SOZO Ministry, have voluntarily agreed to pray for me using various inner healing methods and when appropriate, Pastoral counselling. I understand that this SOZO session is not a professional counseling meeting and that none of the team members are professional licensed counsellors. I understand that these team members are, to the best of their ability, doing what they can to help me achieve more spiritual freedom in my life. *
PLEASE NOTE: Anything that will be divulged in the SOZO session, that is illegal, will need to be reported to the relevant Legal Authority. As Church counselors we are mandated to do so by the Laws of South Africa. *
