This form is to be filled out by guardians of SPS preschool-age students who are interested in preschool placement for their child. Please fill out one form for each preschool-aged child in your home. Please complete this form ONLY ONCE per child.

We are currently accepting entries for Wonder Years and LEAD Academy for the 2023-2024 school year.

To be considered for Wonder Years placement in the 2023-2024 school year, children must turn four years old prior to August 1, 2023.

All potential Early Childhood students must have a free developmental screening through Parents as Teachers; and must reside within the boundaries of Springfield Public Schools. If your child has not been screened, please call the Parents as Teachers office, (417) 523-1333, to schedule a free screening.
