This form is being used to help our families and Westlake Charter School align to the SCPHD COVID-19 Guidelines as stated in their Is it COVID-19 guidelines provided to us in August 2023 .

When filling out the following form, please keep in mind the following:
1) Since contact tracing protocols and return to school timelines might be different for each student, please submit a separate form for each of your students.
2) Please keep your students home if they are demonstrating any flu-like symptoms, such as fever, cough, runny nose, chills, nausea, etc.

IMPORTANT: If your student has tested positive or is exhibiting any symptoms of COVID-19 even with a negative test, please keep them home until you have heard from our contact tracing team with a return to campus date.
