2020 DC Secondary STEM Fair: School Project Registration
This is the official registration form for the 2020 DC Secondary STEM Fair, to be held on Saturday, March 14, 2020 at Dunbar High School.

WHO CAN PARTICIPATE: Students from DC public schools, public charter schools, private and parochial schools, and home schools may participate. Each DC school has the opportunity to register up to three projects for the STEM Fair, with a maximum of three student participants per project.

WHO COMPLETES REGISTRATION FORM: Each school should have one staff member acting as the "School STEM Fair lead." The school lead will submit ONE registration form on behalf of the school, which will include the details for up to three projects, as well as details for all student participants.

INFO NEEDED: This registration form will ask for project titles, project abstract (summary of 250 words maximum), student names, grade levels, parent/guardian emergency contact info, and Universal Student Identifier (USI) numbers. (NOTE: The USI is a 10-digit number assigned by OSSE to each student attending a DCPS or public charter school. You can find the USI for a student within your school's Student Information System (SIS) or by asking your school registrar. Private or home school students are exempted.)

MEDIA/MEDICAL RELEASE FORM: In addition to the registration form, all student participants will need to have a media/medical release form signed by their parent/guardian. School leads are responsible for sending the media/medical release form to each parent/guardian, and ensuring it is completed. School leads are encouraged to send an electronic copy of this form to each parent/guardian via email or text message, where the parent/guardian can complete and submit the form electronically.
