D. References - provide 3 Employment References

Section 3. BT/RBT Job Description and ScoreCard - Review and Acknowledgement

Section 4. Background Screening and Consent Forms

PLEASE NOTE:
Background Check #3 and #4 require further action - Notary signature and Fingerprinting appointment. They will not be attached on the first submission of this Application. Please proceed with completion and submission as soon as possible. You will have access to attach/provide those remaining forms once they are complete.
