In order to give you an opinion about whether your divorce would be recognized in Ontario, we need to collect some information from you using this form.  Completing this form should take about 5 minutes.

The fee for the opinion letter is $249 plus taxes. 

This form must be filled out by the person who is named in the divorce decree and who wishes to get married in Ontario.  

All questions are required.  The form is case-sensitive, so be sure to make the first letter of proper names in uppercase.

You will need to give us some information found on the divorce decree.  You should have it in front of you before you begin.

Once you complete this form, you will get an email with our preliminary opinion, an invoice for our services, and instructions regarding sending us your documents and receiving our opinion letter.

Full name of your current partner who you plan on marrying. *

Information about the Divorce Decree
You will need a copy of the divorce decree in front of you to fill out this information.
Jurisdiction that Granted the Divorce Decree. *
Include the state, country, or province that granted the divorce.  This will appear on the Divorce decree.  Do not include the name of the court.
Official Name of the Court/Authority that Granted the Divorce Decree
I.e. "The Superior Court of California".   This will appear on Divorce Decree
Date Divorce Granted *
The date the divorce decree was signed by the court or authority granting the decree.

Were YOU ordinarily resident, in the jurisdiction that granted the divorce, for at least one year before the Divorce Application was started? *
"Ordinary Residence" means the place where a person resides in the ordinary course of his or her day to day life, or in other words a person's "real home". A person usually will have only one place of ordinary residence.  An ordinary residence depends on physical presence in a place for an extended and regular basis as well as an intention to live there on a more or less regular basis.  "Jurisdiction" means the state or province that granted the divorce.
