PART 1: The IHSAA Pre-Participation Physical Evaluation Form MUST BE PRINTED from our web site and received as hard copy only. The IHSAA form includes a page that a PHYSICIAN ONLY (NP signature not accepted) must complete. Physical must be completed on/after April 1, 2018.
PART 2: This electronic document fulfills requirements including:* Code of Conduct* Concussion and Sudden Cardiac Arrest Acknowledgement* Athletic Fee Program for Athletics/Activities* Parkview Sports Medicine - Consent for Athletic Training Services
PART 3: The Athletic Fee for Grades 9-12 of $80.00 or the Athletic Fee for Grades 7-8 of $50.00 will need to be payed one week prior to the first scheduled team event. (Please make checks payable to Whitko Jr/Sr High School for the $80 or $50 fee ONLY.) Any other fees for lunches, team socks, apparel, etc. should be on a separate check.
