OHS Temporary Parking Permit Application 2020-2021
Fill out this form completely and submit the required documentation electronically. This is for a "Temporary" parking permit.  This permit will only be valid during the hybrid learning schedule.  If/when OHS returns to full-time in person learning for ALL students, the temporary permits would no longer be valid and will need to be turned in to the Deans' Office. Any incomplete applications will NOT be considered for a parking permit.  Documentation (copy of driver's license and proof of insurance) needs to be sent to ohsdeansoffice@sd308.org.
