INSTRUCTIONS AND NOMINATION FORM

Q. Who can nominate a teacher?
A. Students, Parents, or Staff

Q. Who is eligible to be nominated?
A. Any teacher or aide who hasn’t been awarded as teacher of the month for the current year

Q. Can I nominate someone who was Teacher of the Month previously?
A. Yes, you can nominate anyone who has not been a recipient in the current year

Q. If the person I nominated was not chosen for the month, do I need to nominate him/her again?
A. No, your nomination will be reviewed at every committee meeting during the year. If you have additional information to give on the person, you can nominate again.

Q. Does it have to be in essay form?
A. No, your submission can be in bullet-form, but we do ask for specific examples. You also may ask an Academic Excellence Committee Member to contact you (please indicate on the form)
