Student athletes are expected to travel to and from events via school transportation. However, in an emergency situation, a student athlete may need to be transported by a parent/guardian or a designated adult that the parent/guardian signs off on. In such emergencies, coach approval must be obtained and this form must be completed prior to the student athlete leaving with the approved adult. Student-athletes may only ride with their parent/guardian, or named adult on form and the parent/guardian accepts all responsibility in providing this transportation for their student athlete.
