SOAR Cohort Signup
SSI/SSDI Outreach, Access, and Recovery (SOAR) is a program designed to increase access to SSI/SSDI for eligible adults who are experiencing or at risk of homelessness and have a mental illness, medical impairment, and/or a co-occurring substance use disorder. 20 CEUs from the NASW are available for those who successfully complete the course. The self-guided SOAR online training takes case managers step-by-step through the SSI/SSDI application process. Sign up to join a cohort of other Lake County case managers in this important training, led by a local SOAR expert.

The training will last from May 18th through June 29th. The cohort will come together for conference calls weekly to discuss the course content and technical questions. The schedule of classes and conference calls can be found below.

If the call schedule and times conflict with your availability, please email to discuss scheduling. To learn more about SOAR, follow this link:

Upon registering, you can expect to receive an email welcoming you to the cohort and calendar invites for the upcoming calls. Thank you for your interest.
