Gifted and Talented Cognitive Testing Nomination Form
This nomination form is for families seeking Gifted and Talented (GT) identification and/or placement at a Highly Gifted Magnet Site. This application is also available online at http://gt.dpsk12.org. All nominations received from December 2, 2022 through April 7, 2023 will be scheduled for our spring testing window.

The spring testing window for in-district students will occur at a child's school between April 14-May 12, 2023. Out of district/home/private school students will be scheduled to test at the Emily Griffith Campus (1860 Lincoln) by appointment (information to come at a later date). Testing will not be available prior to this window.
Please note: Students cannot be tested twice with a cognitive test in one calendar year (For instance, a student tested in the spring of 2022 would not be eligible to test again until the spring of 2023). During the fall semester, DPS students enrolled in Kindergarten, 2nd, and 6th grades are tested with the NNAT as part of the universal screening process; those families do not need to submit this form unless their child was not assessed during the fall screening.

Students already designated as magnet eligible or HGT will not need to be assessed with further cognitive testing as this data point has been achieved. GT identified and talent pool students may be assessed for School Choice and identification purposes. Please contact your school's GT teacher with any questions regarding your child's designation.

We cannot test Pre-K students younger than age 4. If you are interested in ECE services, please contact the ECE Department.

Non-DPS students (applies to private, home, or out of district), please go to Part IV of the nomination form for more information.
