Complete this form if you have noticed an error on your schedule.  

The following circumstances are considered errors:

• Mistake in level placement (grade level or class)

• Schedule is incomplete

• Too many courses are scheduled

• A required course is omitted

All requests will be reviewed by administration for approval. This form MUST be completed with your parent/guardian.    

**Please note that requests for a different teacher or class period will not be honored**
