September 5-6, 2019 Members Council on Library Services Meeting Registration Form
A regular meeting of the FLVC Members Council on Library Services will be held Thursday and Friday, September 5 and 6 at the State College of Florida, Manatee-Sarasota in Bradenton. We anticipate meeting from 1:00 - 5:00 p.m. on Thursday and from 8:30 a.m. to Noon on Friday.

Please register your attendance below. If attending in person, there is a $35.00 meeting registration fee. Payment may be made by check or purchase order by remitting payment to: University of West Florida, 11000 University Pkwy, Cashier's Office, Pensacola, FL 32514. Checks should be made payable to the University of West Florida/Florida Virtual Campus. An invoice will be sent to you via email.

NOTE: If you choose to pay with cash or a personal check, a mandatory 7.5% sales tax will be assessed.

FLVC accepts major credit cards as a form of payment. You will see this option once you have submitted your registration. Once paid, an automated receipt will be generated from the University of West Florida.

If attending in person, by submitting this registration form you agree to pay the registration fee regardless of whether you attend unless you cancel or transfer your registration.

NOTE: The last day to cancel and receive a refund is Thursday, August 29th.

FLVC Refund or Transfer Policy: Registration can be transferred to colleagues within the same institution or registration can be refunded when cancellations are made within 7 days of the meeting. The last day to cancel is Thursday, May 30, 2019. You can also pay your registration by transferring payment from a meeting that you were unable to attend or that was canceled.
