Food substitutions can only be made if the student attends a LRSD School and has a Physician Signed Meal Modification Form is on file in our office. The allergy will be verified by the Nutritionist. If your child has a form on file, please list the student(s) first and last name AND the ID# or school your student attends. Please call 447-2463 with questions about allergies. (Please leave blank if this does not apply)
