Private Lesson Interest form 2026-2027
Please use this form to express your interest in private lessons for 2024-2025.  Summer Lessons are June and July.  School year Lessons will run from August-May with breaks following the school year calendar.  You will be contacted if/when a spot is available.  ***UPDATE*** As of 7/7/25- slots are extremely limited.  Please fill the form out but know that there may be a waiting list.  We will be in touch very soon!

I understand that invoices are emailed out monthly and should be paid either weekly or monthly but by the end of that month.  Lessons are $100 per month.  This is a monthly tuition rate, not fee per lesson.  If multiple lessons are missed at the fault of Polyphony, we will credit your account.  We do make every effort to schedule make up lessons. *
When there are 5 weeks in a month, this lesson is considered a make up lesson for any that you have missed or may miss in the future.  You will not be charged extra for that month.

I understand that I need to let Robin or Polyphony staff know as soon as I know that I will need to cancel/reschedule a lesson. AND failure to do so 2 hours prior to my lesson will result in a "no show" charge. *
I understand that 3 "no shows" will result in my lesson spot being offered to the first person on the waiting list. *
