This form will be used in conjunction with the protocol for self-rating on the two elements from our MOI that you selected. We will be using your evaluation to help pair teachers for instructional rounds based on the areas of tier I instruction you have identified as an area of growth.
We will not share your personal ratings with others - this form is a tool to collect some data and make meaningful groups for our next round of professional learning from those in our own building.
