Please complete the form if you intend to try-out for Baseball, Lacrosse, Soccer, Softball, Tennis, or Track.  The Athletics Department will use this information to plan tryout procedures and protocol for each sport at their respective location.  You will not be able to tryout until this survey is completed and you are verified as eligible to participate.
