Please complete the following registration form to register your team for the basketball tournament.
The Chicago Westside Branch NAACP, the MAAFA Redemption Project, the Rho Mu Mu Chapter of Omega Psi Phi Fraternity, Inc are hosting the Westside Men's Health Basketball Tournament.  This tournament will take place as part of the Juneteenth West Fest at Columbus Park.

11am - 3pm
Columbus Park Fieldhouse
500 S. Central Ave, Chicago IL 60644

Tournament will begin promptly at 11: 00am
Teams need to be present at 10:30am to check in and find out their tournament slot.

Guidelines :

1.  All members of each team must be 18 years and older.
2. Teams must submit registration and complete payment by deadline of June 3rd.
3.  Each game will consist of two 16-minutes halves (32 minutes total)
4.  At least one member of the team (Captain or Coach) must attend the Coach's Meeting
5.  There will be a cash prize for the winning team.

You will all be required to sign waivers, etc.

Spots are available for the first teams to register and pay the registration fee either online or in person.
